Dying teen attends last dance

APPLETON, Wi. - A Wisconsin teen with a deadly spinal disease is choosing to die on her own terms, but, not before holding a big celebration.

14-year-old Jerika Bolen has spinal muscular atrophy.

The disease leaves her in almost constant pain and without use of the majority of her muscles.

Jerika has now chosen to take herself off the ventilator that's keeping her alive.

Multiple surgeries have failed to relieve her daily pain.

On Friday, Jerika hosted her own prom and was even crowned prom queen!

Hospice care plans are now in place and Jerika will be taken off her ventilator at the end of august.
